Sir, I wish to renew my policy (no. N 578001), date of issue is 13-4-19…, for which I could not pay the regular premium for the last two years (January 19… to December, 19…) owing to financial hardship. I am ready to send a cheque for the amount of total premiums delayed and suffer the general penalty whatever it may be.
